2 - CREDITOR INFORMATION
BACK TO ON-LINE FORM
List in this form, all Creditors to whom you owe money. This form will then be used to prepare your bankruptcy schedules. You must list all of your debts in the bankruptcy schedules you file with the Bankruptcy Court. You are prohibited under penalty of perjury from listing some of your Creditors while selectively excluding others. Include the following types of debts: HOME MORTGAGES, HOME EQUITY LOANS, CAR LOANS, STUDENT LOANS, CREDIT CARD AND DEPARTMENT STORE ACCOUNTS, TAX LIABILITIES, MEDICAL BILLS, RETURNED CHECKS, UTILITIES, FINES AND PENALTIES, UNPAID RENT, ALL OTHER DEBTS. If you have a credit card, but owe no money on it, it is not necessary to list it.
NOTE: ALTHOUGH YOU MUST DISCLOSE ALL OF YOUR CREDITORS, YOU CAN CONTINUE TO PAY THEM AFTER YOU FILE BANKRUPTCY. YOUR ATTORNEY WILL ASSIST YOU IN NEGOTIATING A REAFFIRMATION AGREEMENT WITH THE CREDITOR YOU DESIRE TO CONTINUE TO PAY.
HELPFUL TIP: Before you begin, organize all creditor data you have available to you such as billing statements, payment books, collection notices and other similar documentation. If you have no documentation, call the creditor and ask them to mail you the information. Fill in whatever information you have (even if it is only the Creditor name).
Note: This form will re-appear after you submit your creditor information so that you may continue to add additional creditors. After you submit the last creditor you can review what you have filled out by hitting the browser back arrow.
Important do not submit more than one name at a time in the (Your Name) box, if you try two names using an '&', the form will not submit correctly.
