1- We can provide easy to use online forms for you to have the convenience of submitting much of the information online. You need to call 801-501-0100 and schedule an appoint to determine the chapter you would like to file. We will schedule an appointment and discuss your options. The only form required for the initial appointment is the client questionnaire. Click here for the form and submit it before your appointment.

2- Complete Online Forms:
At the conclusion of the initial consultation you will be shown the process for using our online forms for the rest of the information required. These forms are necessary for the preparation of your case. To initiate use, we will send an email invitation that enables you to log-in to a secure account and complete a series of questions. You may at any time stop and save the work, or chose to submit “Submit to Attorney” when completed. You will be given instructions on what to complete on the forms you are given a password for. Part of the information is imported directly from your credit report so you will be given that information at the time of your appointment to review, so only names not on the report need to be added. Also an expanded Personal Property form can be accessed from this link and you can fill it out and submit it in addition to the forms you will be given a password for. The link is Personal Property
